Antifosfolipiden antistoffen (APLA)

Targets: bèta2-glycoproteïne-1 en andere stollingsfactoren

APLA aantonen

Answer 2

• lupus anticoagulans test: zorgt normaal voor stolling, maar nu niet
• anticardiolipine antistoffen
• antistoffen tegen bèta2-glycoproteïne-1 - Apo-H

Question 3

Antifosfolipiden syndroom

Answer 3

Minstens 1 icm APLA:
- arteriële en/of veneuze vruchtdood
- recidiverende vruchtdood door bloedingsstoornissen vd placenta
vaak ook trombopenie, livedo reticularis
- verhoogde kans op vruchtdood, pre-eclampsie en trombose

Behandeling

Answer 4

• reguliere antistolling zoals coumarines en heparine
• trombocytenaggregatieremmers zoals aspirine
• Ig therapie (rituximab)
• plasmaferese
• bij zwangerschap LMWH (kan niet over placenta)

• primair APLS: geen andere auto-immuunziekte erbij
• secundair APLS: andere auto-immuunziekte erbij
• APLS bij lupus-like syndroom: 1-3 vd LED-criteria erbij
