Anaplasma

A

Anaplasmosis - transmitted by Ixodes tick, which live on deer and mice.

Causes granulocytes with morulae in their cytoplasm. Present in south and west where Ixodes is found. Severe anemia and wasting in diseased cattle. Hematuria secondary to red blood cell lysis.

A

Cat scratch disease - Gradual lymph node enlargement (axilla, groin, neck) after distal scratch or red-brown skin papule.
Bacillary angiomatosis - vascular proliferative disease involving mainly skin and other organs. Associated with HIV or immuno incompetence.

Brucella

A

Brucellosis / undulant fever - unpasteurized dairy.

Gram negative, small nonmotile coccoid rods. Transmitted by ingestion or direct contact with infected animals / abortion or premature birth, excreted in urine, milk, placenta. Non-caseating csarcoid like granulomas.

Treat with doxycycline

A

Psittacosis - parrot fever. Transmitted by parrots and other birds. Remember: Parrots like pistacios.

Coxiella burnetti

A

Q fever. Aerosols of cattle / sheep amniotic fluid. Small gram negative, similar to Rickettsia. No arthropod vector. Tick feces and cattle placenta release spores that are inhaled as aerosles. Presents as pneumoniea.

Remember: Q fever is queer because it has no rash or vector and its causative organism can survive outside inits endospore form.

A

Ehrlichiosis - lone star tick.

Monocytes with morulae (berry like inclusions in the cytoplasm). Rash is rare.

Leptospira

A

Hawaiian surfers!

Causes Leptospirosis - Animal urine, contaminated water or soil, infected tissue.

Found in water contaminated with animal urine causing leptospirosis, flu-like sx, jaundice, photophobia with conjunctival suffusion (erythema without exudate). Prevalent among surfers and in the tropics (Hawaii).

Rickettsia rickettsii

A

Rocky Mt Spotted Fever - Dermacentor ticks

Occurs in North carolina and south atlantic states.

The bugs need CoA and NAD+ because they cannot synthesize ATP!!!

Rash typically starts at wrists and ankles and then spreads to trunk, palms, and soles.

Classic triad - headache, fever, rash / vasculitis.
PALMS AND SOLES seen in 
Coxsackie A - hand, foot, mouth disease
Rmsf 
Secondary syphilis

Endemic typhus - Fleas

Rash starts centrally and spreads out, sparing palsm and soles..

Yersinia pestis

A

Plague - Fleas - rats and prairie dogs are reservoirs.

Bacteria forms a biofilm that blocks digestive tract of the flea so that it keeps on biting you.
