Method of Fixation: Cement; WB status?
Immediate WBAT
Method of Fixation: Cementless/hybrid, WB Status
Partial weight bearing/toe touch for at least 6 weeks or or WBAT immediately after
(range)
Common resting position of the hip while patient is in supine.
Slight hip abduction with neutral rotation
Motions to Avoid with the Posterior or Posterolateral Approach?
- Hip Flexion > 90 degrees
- Adduction beyond neutral
- IR beyond neutral
Transfer direction for Posterolateral Approach?
Transfer to the sound side
Sitting recommendations for the posterolateral approach?
- Avoid sitting on low chairs
- keep knees slightly lower than hips when sitting
- Use raised toilet seat
Rotation recommendations for the posterolateral approach?
Avoid standing activities that involve rotating the body to the operated side
(Want to rotate towards non-operated side)
Motions to avoid: Anterolateral Approach?
- Hip Flexion > 90 Degrees
- Extension Past Neutral
- Adduction Past Neutral
- ER past Neutral
Stepping pattern recommendations for the anterolateral approach?
Early Ambulation:
- Step to rather than past the operated hip to avoid hyperextension
Anterolateral Approach rotation recommendations?
Avoid activities that involve standing on the operated side and rotating away from the involved side.
(Rotate towards involved side; prevent ER)
Utilizing the Posterolateral approach allows what muscle to be spared? Caveat to this approach?
- Allows the abductor muscles to remain intact
- High incidence of postoperative instability secondary to interruption of the posterior joint capsule
When will a direct lateral approach be utilized over the posterolateral approach?
Patients who may be noncompliant with movement precautions
Main indication for a cementless fixation?
Young active individuals (less than 65 y/o)
Trunk Motion Considerations?
Avoid excessive leaning forward during any activity
- bending over to tie shoes
- reaching forward in bed to pull up covers
