What is the Pharmacology of Tenecteplase
A
Fibrinolytic - A modified form of tissue plasminogen activator (tPA) that binds to fibrin and converts plasminogen to Plasmin

What is the Presentation of Tenecteplase and how is it Metabolized

Presentation
- 50mg in a pre-filled syringe containing water for injection
Metabolized
- by the liver

What are the Indications and Contraindications of Tenecteplase
A
Indications
- Acute Stemi
Contraindications
- Major Surgery in the past 3 months
- Significant head injury in the last 3 months
- Major Trauma in the last 3 months.
- Stroke/ TIA in the last 3 months
- ICH at any time
- GI or GU in the past month.
- Current Bleeding Disorders active bleeding (excluding menses) or bleeding tendencies.
- Anticoagulants or glycoprotien IIb/ IIIa inhibitors
- Allergy to Tenecteplase or gentamicin

What are the Side Effects and Precautions of Tenecteplase
A
Side Effects
- Bleeding, including at injection site, ICH or internal bleeding
- Transient Hypotension
- Allergic reactions including (Rash, Fever, Chills, Nausea, Bronchospams, Vasulitis, Nephritis and anaphylaxis)
- Cholesterol Embolism
Precautions
- Age > 75
- Non compressible vascular puncture.
- History of Liver disease.
- Systolic BP >160 or DBP > 110
- Low body weight
- Active peptic ulcer.
- Anemia
- Traumatic or Prolonged (> 10mins) of CPR
- Pregnant or within 1 week post postpartum
- HR > 120 Bpm.
- Acute pericarditis or Sub acute bacterial endocarditis.
