Spinal Stenosis / Neurogenic Claudication Types

A
  • Neurogenic = associated with spinal stenosis, symptoms manifested in back or sciatic nerve distribution, sometimes called pseudo claudication or cauda equina syndrome
  • Vascular = common result of arteriosclerosis, arterial embolism, or thrombo-angitis

Bicycle Test of Van Gelderen

A
  • Patient seated on exercise bike and asked to peddle against resistance
  • Patient started pedaling while leaning backward (if pain to butt and post thigh and tingling in affected LE then this part is +)
  • Patient then asked to lean forward while continuing to pedal (if pain subsides over short period of time then test is +)
  • If patient sits upright again, pain returns
  • This test determines whether patient has neurogenic intermittent claudication
