Upper SI obstruction
Acute presentation
Large volumes vomited
Lower SI obstruction
Colicky abdominal pain
Distension
Vomiting - possibly faeculant
Vomiting
The more proximal the obstruction the earlier the vomiting develops
Thick, brown foul smelling vomit
Lower SI obstruction
Copious, bile stained fluid
Upper SI obstruction
Colicky pain
Occurs as peristalsis tries to overcome the obstruction
Distension
Caused by swallowed air and intestinal fluid secreted proximal to the obstruction
Investigations
AXR
CT scan
AXR
Bowel proximal to obstruction is distended with gas
Distended small bowel loops (centrally)
CT scan
To confirm diagnosis and look for a cause
“cut off” observed between dilated proximal and collapsed distal
