What is scarlet fever?
A
- reaction to erythrogenic toxins produced by Group A Strep. (Streptococcus pyogenes)

A
- more common aged 2 - 6 years
- peak incidence = 4 years.

Transmission of scarlet fever
A
- Respiratory route
=> inhaling/ingesting respiratory droplets
=> direct contact with nose and throat discharges

2-4 days

Clinical features of scarlet fever
A
fever: lasts 24-48 hours
malaise, headache, nausea/vomiting
sore throat
‘strawberry’ tongue
rash - starts on torso, spares palms/soles

A
- fine punctate erythema (‘pinhead’)
- flushed appearance
- circumoral pallor
- rash more obvious in flexures
- rough ‘sandpaper’ texture
- desquamination later particularly around the fingers and toes

A
- Oral penicillin V for 10 days
- Pen allergic = azithromycin
