What should the nurse assume when a patient is in restraints
A patient in physical or mechanical restraints may be confused or delirious and will probably be frightened at the limitation of movement
Should not assume they understand the need for restraints
Support and reassurance for restraints
Should be spoiled efficiently with care not to injure patient
Padding of cuff restraints help prevent skin breakdown
Patient should be positioned in anatomical alignment to prevent skin breakdown
Privacy for restraints
Is essential
If visitors are allowed, explain reasoning for restraints
Face to face assessment within a hour after placement is required
Physical needs while in restraints
Vitals should be checked
Regular observation of circulation in extremities
Fluids should be offered regularly and opportunity for elimination
Skin care is essential
Restraints should be released every 2 hours to allow exercise
