Large: instantaneous death, CV collapse, haemodynamic compromise (if obstructing >60% of the pulmonary circulation)
Medium: haemorrhage or wedge-shaped infarction (less common, due to lung’s dual circulation)
Small: clinically silent or may cause transient chest pain, haemoptysis, small infarctions
