Preeclampsia/eclampsia Flashcards
preeclampsia
A serious and potentially life-
threatening high blood pressure
condition in pregnancy and/or
postpartum
eclampsia
A serious complication of preeclampsia that results in seizures either in pregnancy or postpartum.
spiral artery remodeling 1
Early changes in the vessel wall, with
endothelial cells swelling and individual smooth muscle cells becoming enlarged.
spiral artery remodeling 2
Extravillous trophoblasts (EVTs) from the placenta start to invade the tissue
surrounding the spiral artery, disrupting the smooth muscle layer.
spiral artery remodeling 3
EVTs actively penetrate the vessel wall, entering the lumen of the spiral artery
spiral artery remodeling 4
The invading trophoblasts become
embedded within the vessel wall,
forming a fibrinoid layer that replaces
the muscular wall.
spiral artery remodeling 5
The remodeled spiral artery is re-lined with new endothelium and a thickened subintimal layer containing
myofibroblasts.
spiral artery in pregnancy
extremely dilated to get better blood supply to baby
patho of preeclampsia
Failure of trophoblasts to invade and remodel the uterine blood vessels
preeclampsia cell specifics
- Improper recruitment of
trophoblasts - Improper trophoblast and
cell differentiation - Defective maturation of
endothelial and NK cells
Decreased Placental Perfusion mother response
- Inflammatory Response
- Decreased blood supply to
the fetus
3.Proteins Released - Endothelial Cell Damage
>Increased permeability ->edema
>Narrowing of vessels -> HTN
>Thrombi Formation
>Retention of Na+ in kidneys ->
proteinuria
HELLP Syndrome
Hemolysis,
Elevated Liver
enzymes, Low
Platelet coun
