diag+ Peritonite

A

clinique. urgent.
DAA intense, occlusion, fievre +/- frissons. cri de Douglas/dlr a decompression brutale . defense voir contracture (DEPIT Dlr Permanente Expansive Permanente Invincible Tonique).
+++ IIaire a perfo organes creux.

EComplé péritonite

A

AUCUN si choc.
pas choc = bio = preop, hemoc, NFS-plaq iono CRP cs anesth.
ASP face debout centré sur coupoles = pneumoperitoine+++
scan = ref abdopelv.

PEC peritonite.

A

urgent avis chir : SdG = choc, épanchement stercoral, terrain fragile, ATB non adaptée.
2VVP+ remplissage SNG si besoin, stabilisation constante/hemodynamique.
bloc laparo. prelevements pour microbio, lavage peritoneal, drainage + ttt cause
- suture/appendicetomie/resection.
ATB ASAP proba puis adapté augmentin+genta IV 3-5j ou 7-15j.
+IPP preventifs.

3 peritonites secondaires =

A

perforation = peritonite localisee + signes selon localisation :
- appendiculaire = FID fievre hyperPNN CRP. pas de pneumoperitoine.
- diverticulaire = FIG, pareil + pneumoperitoine++
ulcere = epigastre, brutal+ pneumoperitoine, pas de PNN/fievre. cherche ATCD AINS/tabac/ulcere

primaire = translocation bact, +++ ascite par EColi (C3G), KT (S Aureus) vanco+genta
tertiaire = BMR+++ chir svt impossible ==> rea
