PEC en urgence HD

A

diag+ = éliminer ddiff.
évaluer gravité+++ signes de choc, Hb/Hte sous-estiment HD aiguë.
Hospit, surveiller constantes, 2VVP, RAI, db deter NFSplaq TP/TCA, iono S, + prévoir CG adaptés
remplissage fc100, Hb>8, diurese>30mL/h.

EOGD (après stabilisation) = diag+, localise, diag gravité et geste hémostase.
proteger VAS avant EOGD.
si EOGD nl = artériopathie dig ==> scan.

Ttt HDH sur UGD
initial
préventif

A

initial = hemostase par EOGD + IPP.
- grave = PD IV 48/72h, puis PD PO
- pas grave = PD PO d’emblée
préventif = eviction med gastrotox, eradication HP, IPP long cours ssi AINS non arretables.

initial
préventif

A

initial = vasoactif IV. si rupture VO = + hemostase par EOGD. si cirrhotique = prevention surinf ascite et encéphalopathie. (ATB et laxatifs osmotiques)
TIPS/anastomose portocave = discuté si hémorragie non contrôlable.
préventif = ligature jusqu’à disparition VO +B- a DME pour Fc~55bpm
