Where is acid secreted from and what is this stimulated by? What receptor?
Parietal Cells by on H2 receptors histamine from paracrine cells near to parietal cells.
What stimulates acid production?
ACh through vagal fibres (sympa)
Gastrin from G cells in response to amino acids in the stomach
Gastic distension (local and long reflexes)
What is Ranitidine?
H2 receptor antagonist
How fast is Ranitidine at working?
Rapid when taken orally
What are Omeprazole and Lonzoprazole?
Proton Pump Inhibitors
How long are Omeprazole and Lanzoprazole effective for?
They bind irreversibly making them very long acting as body can only respond by creating new pumps.
What are PPIs useful in the treatment of?
H. pylori
Name a mucosal protectant
Sucrafate- sticks like a gel on ulcer site
How to antacids work?
Increase luminal pH which speeds up gastric emptying
How long are antacids effective for?
Very short term
What is the downside of antacids?
Acid rebound when they are stopped
What compounds are found in antacids?
Sodium bicarb
Magnesium hydroxide
How do you test for H pylori?
Urea breath test
How do you treat H pylori?
PPI and antibiotics triple therapy
Omeprazole and Clarithromycin and Azithromycin
