What are the properties of NSAIDs?
Analgesic
Antipyretic
Anti-inflammatory (high dose)
What is the mechanism of action of all NSAIDs?
COX Inhibitors which means prostaglandin synthesis is inhibited (bradykinin and histamine inactivated)

Ibuprofen Aspirin Naproxen Diclofenac Indometacin
What would you use for inflammatory joint disease?
Ibprofen
Naproxen
What would you use for post-op pain?
Diclofenac
What are the side effects of NSAIDs?
GI tract- COX-1 stimulates PGE 1 and PGI2 which stimulate mucous and bicarb secretion to protect gastric mucosa. NSAIDs block COX 1 leading to bleeding and ulceration. PPIs protective
Nephrotoxic- In the absence of COX 1 there is Na retention and decreased blood flow leading to renal failure (especially in cirrhosis or CCF) as normally it causes vasodilation.
Chronic NSAID use= chronic renal failure
