NTG (Class)
nitrite/vasodilator
NTG (MOI)
causes vasodilation, thereby:
decreases preload, myocardial workload, myocardial oxygen consumption
increases perfusion through coronary vessels to myocardium
NTG (Indications)
chest pain of suspected cardiac origin
respiratory distress of suspected cardiac origin (chf)
NTG (Contraindications)
shock and suspected intracranial bleeding
NTG (adverse reactions/side effects)
transient hypotension (can be profound)
temporary pulsating headache and/or facial flushing
dizziness
NTG (Dosage and Administration)
0.4-0.8 SL q 5 min. x3 (if bp <100 systolic. may repeat prn
NTG (Special information)
check bp before and after
prepare patient for administration (start IV if BP is borderline, but not required prior to administration)
store in dark container and replace opened contained every 30 days
assess effectiveness by patient reassessment
if hypotension devlops, consider fluid challenge
some brands do not cause bitter taste under tongue as in past. not a reliable indicator of efficacy
ask patient about ED drug use and date/time taken
not NTG should be given under SO to a patient who has taken viagra (sildenofil) or levitra (lardenafil) within 24hrs or cialis (tadalafil) within 48hrs
if patient is on viagra, etc, and OLMC cannot be reached, start IV and give small doses of morphine with prn fluid boluses instead of NTG
use with caution in patient with inferior/right side MI
