dopamine (class 2)
sympathomimetic (naturally occurring catecholamine)
vasopressor with dopaminergic, alpha, and beta adrenergic properties
dopamine (actions 4)
acts on alpha/beta adrenergic receptors as well as on dopamine receptors
at dosages from 1-10mcg/kg/min: acts primarily on beta receptors causing positive inotropic effect on the heart. dilates blood vessels supplying brain, kidneys, heart, and GI
at dosages greater than 10mcg/kg/min: begins to stimulate alpha receptors, resulting in peripheral vasoconstriction
increases systolic bp and pulse pressure
dopamine (indications 5; 1,2,5,1,1)
hypotension associated with chest pain (suspected cardiogenic shock)
anaphylaxis in extremis (severe hypotension, severe respiratory distress) if pt still has hypotension after epi and fluids
symptomatic bradycardias with a pulse (chest pain, SOB, decreased LOC, BP less than 90 systolic, pulmonary congestion) after atropine
respiratory distress of suspected cardiag etiology (chf)
shock/hypotension
dopamine (dosage/route)
400mg/250ml NS at 2-20mcg/kg/min titrated to systolic bp and cardiac rhythm response
dopamine (side effects 2)
ventricular irritability
hypertension & extreme vasoconstriction can occur with high infusion rates
