A
  1. normal: alert, with a normal response to environmental stimuli
  2. confused and disoriented: responding to environmental stimuli in an inappropriate manner
  3. Depressed: drowsiness, inattention and less responsive to environmental stimuli
  4. stuporous: state of unconsciousness with reduced responses to external stimuli but can be roused by a painful stimulus
  5. Comatose: state of unconsciousness with absence of response to any environmental stimuli including pain
