protein intake 4-18 y/o
10-30% of total caloric intake
Fat intake 4-18 y/o
25-35% total caloric intake
3 types of sports activities classified by AAP
contact and collision
limited contact
noncontact
objective of sports physical
- cardiovascular risks
- health status: fitness level
- detecting injuries, conditions, and illness that might limit competition/lead to injury
- recommend alternative activities as appropriate
- anticipatory guidance about safety equipment
- promote healthy choices
- recommend ways to improve athletic performance
components of pre-participation physical exam
- Ht/Wt and BMI
- BP/HR
- general appearance: marfan’s
- eyes: eyes should be > 20/40 corrected
- CV
- respiratory: accessory muscle use, prolonged expiration
- GU: hernias, vericoceles, undescended
- Musculoskeletal
- Skin: molluscum, HSV, impetigo, MRSA, tinea corpis, scabies
- neuro: gross motor assessment
CV screening PMH
- chest pain/discomfort related to exertion
- unexplained syncope
- excessive exertional and unexplained dyspnea/fatigue/palpitations with exercise
- prior heart murmur
- HTN
- prior restriction from participation in sports
- prior testing for the heart
cardiovascular screening FHx
- premature death, before 50, r/t heart disease
- disability from heart disease in close relative <50
- hypertrophic or dilated cardiomyopathy
- long QT syndrome
- marfan syndrome
- clinically significant arrhythmias
Causes of sudden cardiac death in young athletes
hypertrophic cardiomyopathy
anomalous origin of a coronary artery
-> ventricular arrhythmia
Assessing young athletes for head injury
- baseline neurocognitive testing
- concussion may occur with no obvious indications of concussion
- no return to play same day of injury
- graduated return-to-sport strategy should be used after concussion
- 6 steps, each should take at least 24 hours
Female athlete triad
- low energy availability with or without disordered eating
- menstrual dysfunction
- low bone mineral density
Associated with: - sports that emphasize a lean body mass,
- low wt maintenance
- retaining ones prepubertal physique
Concussion
mild traumatic brain injury
- complex pathologic brain process resulting from primary or secondary forces on the head that disrupt brain processes and functioning
- > physical, cognitive, emotional, and sleep s/s
Concussion exam physical
headache nausea/vomiting difficulty with balance changes in vision dizzy light or sound sensitivity paresthesias dazed or stunned
concussion exam cognitive
confusion altered concentration mental torpor altered memory forgetfulness - conversations or recent events needs to repeat or slowly answer questions
concussion exam emotional
abnormal irritability
feelings of sadness or being “emotional”
abnormal feelings of being nervous
concussion exam sleep
drowsiness
insomnia or hypersomnia
difficulty falling asleep
