- dose administered by wrong mode of treatment
- leaking sealed sources
what is the course of action to take, if a medical event were to occur?
- within 24 hrs report to patient and doctor
- notify NRC operations center by phone by the next calendar day after the event was discovered
- write a written report to NRC w/i 15 days after discovery of medical event
