Lacrimal System And Dry Eye Flashcards
1
Q
the tear film is composed of three layers secreted by ___
A
- outer layer (Lipid) by meibomian
- middel (Aqueous) by lacrimal
- inner (Mucin layer) by goblet cells
2
Q
can you enumerate the lacrimal system disorder
A
- acute / chronic / infantile dacryocystitis
- acute / chronic dacryadenitis
- dry/watery eye
- Canaliculitis
3
Q
excessive lacrimation (watery eye) called
A
Epiphora
4
Q
Congenital naso-lacrimal duct obstruction called
A
Congenital or infantile dacryocystitis
5
Q
infantile dacryocystitis caused by
A
mainly by obstruction of hasner’s valve
6
Q
clinical features of infantile dacryocystitis
A
- epiphora
- recurrent conjunctivitis.
- Pressure over the lacrimal sac leads to reflux of clear fluid, mucus, muco-pus or sometimes pus.
7
Q
treatment of infantile dacryocystitis
A
- conservative (antibiotic + massage) for 6 months
- Probing carefully [1]
- Repeated syringing & irrigation (curative)
- Silicone intubation
- Dacryo-Cysto-Rhinostomy (DCR)
[1] lacrimal sac still elastic
8
Q
in Acute Dacryocystitis regagitation test is +ve o -ve?
and why?
A
-ve.. congestion of the epithelium of canaliculi.
9
Q
complications of Acute Dacryocystitis
A
- Lacrimal fistula: The sac may burst anteriorly through the skin.
- Pyocele: Canaliculi may become obstructed.
- Orbital cellulitis and cavernous sinus thrombosis.
- Chronic dacryocystitis.
10
Q
treatment of Acute Dacryocystitis
A
- During the acute phase:
a. Antibiotics: Systemic and topical.
b. Hot fomentations.
c. Lotions: To clean the pus.
d. Incision and drainage if an abscess forms. - After the acute attack subsides: Dacryocystorhinostomy with
fistulectomy if needed.
11
Q
causes of dry eye
A
- abscence of lacrimal gland
- inflammation of the gland
- tumors
- Keratoconjunctivitis sicca
- Conjunctival scarring (trachoma, chemical burns, Stevens-Johnson syndrome and ocular cicatricial pemphigoid.
12
Q
treatment of dry eye
A
- Protective glasses and contact lenses.
- Tear substitutes (eye drops, eye gel).
- Occlusion of the puncti to reduce tear drainage.
- Systemic steroids (autoimmune cases).
