What are the clinical features of anaphylaxis?
IS AWFUL HARD:
Itch (palms, soles, genitalia)
Stridor (due to laryngeal obstruction)
Angiodema (of lips and mucous membranes) Wheeze (due to bronchoconstriction) Flushing Urticaria Loss of Consciousness
Hypotension
Arrhythmias
Rhinorrhoea
D&V
Once someone has had an anaphylactic reaction, how should the cause be identified?
IgE blood test (skin prick should not be used)
What is “serum sickness”?
is a reaction to proteins in antiserum derived from a non-human animal source, occurring 5–10 days after exposure. Symptoms often include a rash, joint pain, fever, and lymphadenopathy. It is a type of hypersensitivity, specifically immune complex hypersensitivity (type III)
Around how long after exposure does serum sickness develop?
3 days
Recall some symptoms of serum sickness
IgG deposits in:
- Joints (causing arthralgia)
- Kidney (causing AKI)
- Skin (causing a vasculitic rash)
How should serum sickness be managed?
Remove allergen
Corticosteroids
Manage fluids
