Administering before 9AM
What drug interactions should we look out for?
Diuretics, NSAIDs, insulin, vaccines, Digoxin
Contraindications for glucocorticoids?
systemic fungal infections and live virus vaccines
What other groups should we be cautious about giving steroids to?
pediatrics, pregnant or breastfeeding, HF, GI issues, DM, osteoporosis, and current resistant infection
MOA of glucocorticoids?
anti-inflammatory and immunosuppressive
What is the suffix for steroids?
-sone
What are the adverse affects?
4S’s (Swollen, sepsis, sugar, skinny, sight)
adrenal insufficiency, osteoporosis, infection, high glucose levels, myopathy, iatrogenic cushing’s syndrome, Na+ and water retention, K+ loss, growth retardation, psychological disturbances, cataracts and glaucoma, and peptic ulcer disease
How do we prevent adrenal insufficiency?
Tapering down
What are the sx of Cushing’s syndrome?
hyperglycemia, glycosuria, fluid and electrolyte disturbances, osteoporosis, msl weakness, cutaneous striations, lowered resistance to infection, redistribution of fat → moon face, buffalo hump
