What are antitussive opioid options?
Codeine and hydrocodone
MOA of codeine?
suppresses cough through its action on the CNS to increase the cough threshold
Therapeutic use for codeine?
off-label cough suppression
Adverse effects of codeine?
RESPIRATORY DEPRESSION, dizziness, lightheadedness, drowsiness, risk for abuse
What is the prototype for non-opioid antitussives?
Dextromethorphan (Delsym)
Therapeutic use of Dextromethorphan?
cough suppression; can reduce pain when prescribed with an opioid
MOA of Dextromethorphan?
Suppresses the cough center in the medulla
Adverse effects of DextRomethorphaN?
dizziness, sedation, rash, nausea
Contraindications for Dextromethorphan?
concurrent or within 14 days of MAOI use, SSRI use, hypersensitivity
Precautions with use of Dextromethorphan?
- cough that last more than 1 week or is accompanied by a fever, rash, or HA can be a sign of a serious condition and PCP should be consulted.
- Hx of drug abuse or drug seeking behavior.
- diabetes (some drugs contain sucrose)*
Onset of action and route for Dextromethorphan?
onset: 15-30 minutes
route: PO- liquid, lozenges, and syrup
What is the prototype for Expectorants?
Guaifenesin (Robitussin, Mucinex)
Therapeutic use of Guaifenesin?
expectorant; to facilitate removal of phlegm during a cough
MOA of Guaifenesin?
increases volume and decreases viscosity of respiratory tract secretions
Adverse effects of GuaifeNesin?
rash, HA, N/V, dizziness
