Anti-platelets Flashcards
Why are glycoprotein IIb/IIa receptor antagonists considered “super aspirins”?
inhibit the final step in aggregation; prevent aggregation stimulated by all factors.
Uses?
prevent ischemic events in patients with ACS or undergoing Percutaneous coronary intervention (PCI)
Prototypes for anti-platelets used in the hospital?
Abciximab (ReoPro) and Eptifibatide (Integrilin)
Suffix ending for monoclonal antibodies?
-mab
MOA of Abciximab?
Binds near the receptors to block fibrinogen from binding.
How long do effects last after giving Abicixmab?
24-48 hours after stopping the infusion
MOA of Integrilin?
Small peptide that inhibits platelet aggregation by binding to GP IIb/IIa
How much ASA should you take for the prevention of arterial thrombosis? chronic management?
prevention: 325mg or less
chronic management: 81mg or less
Which anti-platelet can be taken at home after having stents placed?
Clopidogrel
MOA of Clopidogrel?
irreversibly blocks receptors for ADP (7-10 days)
Clopidogrel uses?
prevent stenosis of coronary stents and for secondary prevention of MI, ischemic stroke, and other vascular events.
Clopidogrel adverse effects?
TTP
Contraindications and precautions for Clopidogrel?
Contraindications: active pathological bleeding, including ICH and bleeding ulcers.
cautions: when taking proton pump inhibitors (PPI)
How does PPI use effect the effectiveness of Clopidogrel?
Will not work as well
How should Clopidogrel be taken?
1/day with or without food
