Chronic Heart Failure Flashcards
Definition of clinical heart failure
cardiac output is inadequate for the bodies requirements
Poor prognosis of …..% mortality at 1 year
30-40%
Symptoms
SOB
Fatigue
Oedema
Reduced exercise capacity
Sudden death
Signs
oedema
tachycardia
raised JVP
chest crepitations
3rd heart sound
displaced/abrnomal apex beat
Objective evidence of cardiac dysfunction is mandatory because…
heart failure is hard to diagnose on its own
chest crepitations
crackling or rattling sounds
Objective evidence of cardiac dysfunction can be investigated by
ECHO
Cardiac MRI
Radionucleotide Scan
Left Ventriculogram
ECG
BNP
brain natriuretic peptide
elevated in heart failure (low BNP could exclude heart failure)
When first investigating - always look at
ECG
BNP
ECHO
What causes heart failure?
LV systolic dysfunction
valvular heart disease
paricardial constriction
LV diastolic dysfunction
cardiac arrhythmias
Causes of LVSD
ischaemic heart disease
severe AV or MR
(can be inherited, toxic, viral)
Diagnosis of LVSD
detailed history (limes disease? alcohol?)
autoantibodies
Tests for LVSD
Echo
ECG
CXR
CT coronary angiography
LV ejection fraction
continuous biological variable
disease can increase or decrease this variable
Pharmacological treatment of heart failure
Diuretic
ACE inhibitors
Beta blockers
aldosterone receptor blockers
