CHAPTER 5 Flashcards
Data analysis is often referred to as
diagnostic phase or clinical reasoning phase
The end result of the clinical judgment section includes
identification of client concern
collaborative problems
referrals to health care
this is the second step of the nursing process
Data analysis
steps to make a clinical judgement from assessment data
Identify (abno cues;strengths)
Cluster (cues)
Draw (inferences to propose clinical judgements
Identify (possible concern)
Validate (clint concern with client, family)
Document (JUDGEMENT
used to identify actual or a risk for
client concerns.
abnormal cues
are client strengths used in identifying areas where
the client’s health can be improved; they are also used to decrease potential
client concerns
identified supportive cues
are defined as “certain
physiological complications that nurses monitor to detect their onset or changes in
status;
Collaborative problems
can be defined as
connecting clients with other professionals and resources.

3 client concerns
opportunity for health promotion
risk for health concern
actual health concern
occurs when the client
has the motivation to increase or improve their
health behaviors and well-being.
opportunity for health promotion
reflects a state in which the
client does not currently have the problem but is vulnerable to developing
Risk for health concern
indicates that the client is currently experiencing the
stated problem
actual client concern
can occur when the nurse validates and documents
data, includes too many or
inadequate data, unreliable or invalid data, and an insufficient number of cues
available to support the client concerns.
1st set of pitfalls
occurs during the analysis phase. Cues may be
clustered yet unrelated to each other. For example, a client may be very quiet and
appear depressed. A nurse may assume the client is grieving because her husband
died a year ago but the client may just be fatigued because of all the diagnostic tests
she has just undergone.
2nd set of pitfalls
It is often called the
clinical judgment phase because the purpose of this phase is identification of client
concerns
