To calculate 5 year risk and lifetime risk of breast cancer according to the following key risk factors:
1. Age
2. Age at first period
3. Age at first birth
4. Family history of breast cancer (mother, sister or daughter)
5. Number of past breast biopsies
6. Number of breast biopsies that showed atypical hyperplasia
7. Race /ethnicity
8. Mutation in either BRCA1 or BRCA2
9. Hx of ducal carcinoma in situ or lobular carcinoma in situ.
