what are the criteria that indicate ACS ?
1-chest pain lasting longer than 15 minutes , over 20 minutes
2- chest pain with nausea vomiting and sweating
what immediate management should you offer for a patient presenting with cardiac chest pain ?
1- pain relief - so give nitroglycerin
2- aspirin 300 mg
3- pulse oximetry , less than 94 give oxygen
when should you repeat the troponin test ?
10-12 hours after symptom onset
what early management should you offer a patient with ACS ?
300 mg aspirin
what is the first line treatment when a patient is diagnosed with myocardial infarction ?
1- administer antiplatelet loading dose
2- Transfer the patient for urgent percutaneous angiography and angioplasty
what is the treatment for a patient diagnosed with prinzmetal angina ?
Calcium channel blockers, Nitroglycerine and statin treatment Smoking cessation, life style modification
The pain is increases by movement of the chest, cough and walking with no abnormalities seen on ECG ?
pleuritic chest pain
atypical chest pain suggestive of inflammatory disease
what does persistent elevation of ST segment after 1 week indicate ?
reinfarction or a left ventricular aneurysm
management of acute myocardial infarction ?
Administer oxygen via a facial mask
Give the patient aspirin 300 mg and clopidogrel 600 mg or prasugrel 60 mg
Establish IV access and connect patient to cardiac monitor
If patient is distressed or in pain give morphine 5–10 mg IV with 10 mg metoclopramide
Urgent cardiology review for primary angioplasty where available
Otherwise, if patient satisfies the criteria for thrombolysis and has no contraindications, administer thrombolysis with rt-PA
