Aetiology of angiomyolipoma?
80% sporadic: Middle aged female
20% assoc with tuberous sclerosis
Pathology of angiomyolipoma?
Blood vessels
Immature smooth muscle
Fat
Signs & symptoms of angiomyolipoma?
Incidental finding 50% of the time
- Loin pain
- Haematuria
- Mass
- Wunderlich’s syndrome (10%): massive retroperitoneal bleed
Investigations for angiomyolipoma?
USS: Bright echo pattern
CT: Fatty tumour of low density
Treatment of angiomyolipoma?
4cm cut off
Elective: embolization/partial nephrectomy
Emergency: Embolization, emergency nephrectomy
Pathology of oncocytoma?
- Small, oval, well circumscribed
- Mahogany brown with a central stellate star
Histology of oncocytoma?
Very pink and granular cytoplasm
Histology of oncocyte?
Big pink cell
-Fluffy cytoplasm (granular cytoplasm) = packed with mitochondria
Signs & symptoms of oncocytoma?
Incidental finding
Loin pain
Haematuria
Investigations of oncocytoma?
CT: Spoke wheel pattern
Biopsy: Uncertain, similarities to chromophobe RCC
Differential diagnoses for oncoctoma?
Chromophobe carcinoma (why it is taken out)
Treatment of oncocytoma?
Same as RCC
