• Misaligned control rod (greater than 19”)
• Uncoupled control rod
• Reactor Engineering discretion

• 1 or more dropped or misaligned rod with reactor critical and power < 15%
• 2 or more dropped or misaligned rods with reactor critical and power >= 15%
• More than 1 uncoupled control rod
• Unanticipated or anomalous reactor behavior
