Complicated UTI (pyelonephritis)
14 day therapy of ciprofloxacin
uncomplicated UTI
Short term 3 day therapy
Bactrim or ciprofloxacin
TMP/SMX or fluoroquinolone
Pregnant UTI
Beta lactam for 7 days
Augmentin
Cephalosporin
Chlamydia
C. Trachomatis
Drips, urethritis
Sexual or vertical(fetal) transmission
Azithromycin 1g (pregnant) or Doxycycline BID x 7 days
Add 2g metronidazole if persistent
Gonorrhea
Neisseria gonorrheorae
Sexual, perinatal transmission
Purulent urethral discharge
1 g Azithromycin preferred
IM ceftriaxone plus doxycycline BID x 7 days
Pregnant complicated STI
Or allergic to cephalosporin
Use 2 gram Azithromycin
Z-bomb
Do NOT use tetracyclines
Herpes simplex virus
Lifelong viral infection
Sexual and perinatal transmission
Transmission declines with age
1st episode:
Valacyclovir 1g orally BID 7-10 days
Recurrent: Valacyclovir 5 days
Severe or pregnant: acyclovir IV
Syphilis
Treponema pallidum
Sexual and vertical transmission
MSM risk population
Chancres, secondary rash, hair loss
Benzathine Penicillin G IM 2.4 mill
If allergic to penicillin use:
Doxycycline for 14 days
Late latent or tertiary:
Penicillin G 3x 2.4 mill
Doxycycline BID 28 days
Caution jarisch-herxheimer reaction
Erysipelas
Elevated border, group A strept. pyogenes
Procaine penicillin IM
PCN allergy - erythromycin
Cellulitis
Non-elevated, poorly defined margins
Mild - dicloxacillin oral
Severe - nafcillin IV
Polymicrobial w/anaerobes
Mild - Augmentin
Severe - ceftriaxone
Impetigo
Amoxicillin
PCN allergy: erythromycin
Dog or cat bite
Pasturella multocida
Augmentin 875
Diabetic foot infection (multiple organisms
Not MRSA - Augmentin (combo has anaerobe activity)
Mild MRSA - BCD
Bactrim, clindamycin, doxycycline
Severe MRSA (>100.4F) - VLDC Vancomycin, linezolid, daptomycin, ceftaroline
Osteomyelitis
6 week therapy
ABCQ bone penetration
Aminoglycoside, beta lactam, clindamycin, quinolone
First line - beta lactam: Cefazolin IV (or nafcillin IV)
IVDA cipro
IVDA (p. aeruginosa) - ciprofloxacin
Infectious arthritis
Infection within joint space
Duration - 3 weeks
Children - nafcillin IV
Adults - ceftriaxone IV
IVDA - nafcillin (MSSA)
Vancomycin (MRSA)
Cefepime (pseudo. aeruginosa)
