Section A is completed for ALL fire and non-fire occurrences. This section identifies the “where” and “when” of the incident, “who” responded, and to “what” they responded. As well, the section indicates the extent of any rescues or fire fighter injuries or fatalities resulting from an occurrence.
When is Section A and Section D completed for all non-fire responses?

The jurisdictional fire department should file the SIR report that identifies the type of response, and all required information. The assisting department should file a report with the following field complete:

This is a field to indicate whether the fire department contacted OFM investigations with reference to the incident. This is preliminary information based on the time of filing, and does not need to be revised if OFM does not investigate the incident, or the fire department contacts OFM after the Standard Incident Report has been filed.
How is the Fire Department Incident # unique among records filed by a fire department?

This field is defined by the fire department and is used to ensure that OFM records can be matched
back to fire department records. Each report must have a number which is unique among all the records filed by that fire department. There are sufficient columns to include a full 4 digit year
(2009000001) year or a 2 digit year (0900000001) as the start of the identifying number. Using year
in the first two or four columns will assure that the identifier is unique across years.
When reporting the Date of Occurrence, what format should be used?

2 Explosion (including during fire, excluding codes 3 and 11-13)
This code is used to describe an explosion prior to or during a fire, or a combustion explosion that does not result in a fire and may or may not result in a dollar loss. This response type is intended to capture incidents that involve the explosion of combustible solids, liquids, and gases.
Example: “A man suffered burns after his camper trailer exploded because of a propane
fuel leak…. the chief said propane had leaked inside the trailer where the main
was using a barbecue igniter to light the pilot light on the furnace… there was
no fire after the explosion.”
