3-72/REQUEST FOR REIMBURSEMENT FOR LOST OR DAMAGED PERSONAL PROPERTY

Member sustaining loss or damage is responsible for report BP -13 (WHILE IN PERFORMANCE OF DUTY,SATISFACTORY PROOF)

OFFICERS on duty at time of loss or damage will verify LOSS in their endorsement

Question 2

attach original receipt of REPLACEMENT ITEM to report.

amount reimbursed will NOT EXCEED THE PREVAILING PRICE CHARGED BY THE QUARTERMASTER

Member requesting reimbursement must forward property to safety and inspection command

request for reimbursement must be forwarded within 30 days after loss..if member is on Medical leave , from BP -13 must be forwarded within 30 days a full duty/light duty
