5-84 / Evaluation for pROBATIONARY PERIOD

Question 1

EVALUATION REPORT (BP-19A) must be prepared in triplicate at the end of 9th/13th/17th month of service and numbered (e.g. 2,3,4 ) after the date of rating period.

Answer 1

Evaluation beings on date of appointment to FDNY (prior city service does nOT reduce probation period)

• ORIGINAL and copy forwarded to member’s assigned DIVISION
• One copy filed at unit level
• FINAL EVAL must be endorsed by DIVISion commander

Question 2

CC must NOT grant extended leaves, such as vacation, to the 6th grade FF DURING FINAL MONTHS OF PROBATION

ALL EVALUATION reports review and signed by CC or SENIOR OFFICER (WHEN NOT REPLACEMENT FOR CC)

Question 3

When probationary FF’S have their probationary period extended , for whatever reason, additional evaluation reports are required.

For an extension up to 90 DAYS , one report is required. For an extension more than 90 days , a report every three months is required..in all cases the FINAL REPORT is due 4 weeks prior to tenure

When member is assigned to unit upon completion of probation period , OFFICER will received evaluation from FIRE ACADEMY.

Answer 4

If rating is unsatisfactory, member will remain for retraining until a satisfactory rating is earned .
-in no case shall retraining period exceed one complete training cycle
