- primarily caused by ascension of microbs from urethra to bladder

- Males
- infants with congenital defects
- vesicoureteral reflux
- elderly
- enlarged prostate

what does ascension of microbes/UTI to the kidney cause, and how does it present?
A
- ascension to kidney results in pyelonephritis
- presents with:
- fever
- chills
- flank pain
- costovertebral angle tenderness
- hematuria
- WBC casts
- presents with:

what predisposes a patient to cystitis (UTI)?
A
- being a woman
- 10x more common in women b/c shorter urethras colonized by fecal flora
- obstruction
- kidney surgery
- catheterization
- GU malformation
- diabetes
- pregnancy

- Escherichia coli
- Staphylococcus saprophyticus
- Klebsiella pneumoniae
- Serratia marcescens
- Enterococcus
- Proteus mirabilis
- Pseudomonas aeruginosa

A
- leukocyte esterase
- = evidence of WBC activity
- leukocyte esterase
- nitrite test
- = reduction of urinary nitrates of bacterial species (ie. E. coli)

- = urease producing bugs (ie. Proteus, Klebsiella)
