Transplantation Flashcards
what are the three types of donors for transplant?
decreased heart beating donors (DBD)
non heart beating donors (DCD)
live donors
when is malignancy a contraindication for transplant?
known untreated malignancy
solid tumour in the last 2-5 years
what two things need to be matched between recipient and donor?
blood group
HLA
what is the importance of HLA matching in a transplant?
they will be sensitised to that HLA for subsequent transplants
name three sensitising events for a transplant
blood transfusion
pregnancy
previous transplant
what is sensitisation?
the formation of pre-formed antibodies to non self antigens
what is desensitisation?
active removal of a blood group or donor specific antibody
what are the three types of transplant rejection?
hyperacute
acute
chronic
what is hyperacute rejection due to?
preformed antibodies
what is the outcome of hyperacute rejection?
unsalvageable
need another transplant
what is acute rejection due to?
cellular - T cells
antibodies
how can acute rejection be treated?
increased immunosuppression
what is chronic rejection?
antibody mediated slowly progressive decline in renal function
what is given as immunosuppressive induction treatment?
basiliximab
prednisolone during the operation
what three drugs are given as maintenance of immunosuppression post transplant?
prednisolone
tacrolimus
MMF
