What Are the Required Elements of a Valid Prescription?

A

Beginning January 1, 2020 - every prescription must be sent electronically unless exempt:
* Veterinary Rx, Compound with 2+ Components, Jail, LTC, VA, Nursing Home, EpiPen Autoinjectors, Opioid Antagonists, Expedited Partner Therapy, CPA, Bronchodilator Candisters/Spacers, Electronic Failure

Requirements for Rx
* Valid Patient-Provider Relationship
* Date of Issuance
* Patient Name/Address
* Drug/Device Name, Strength, Quanity
* Directions
* Prescriber Name, Address, & Signature
* DEA # (If Required)

Written Rx
* Manual Prescriber Signature (Paper)
* Electronic Signature (Not Required for Non-Controls)

Faxed Rx
* Fax Machine ID
* Fax Time/Date
* Receiving Pharmacy Name, Address, Phone #, Fax #
* If Controlled: Manual Electronic Prescriber Signature

Electronic Rx - includes manual electronic prescriber signature unless exempt:
* Printed/Faxed Paper Rx: Controlled - Manually Sign
* Electronic Rx: Non-Controlled - No Sign Required
* Ensures Electronic Transmission Complies w/ Laws
* Prescriber Provides Verbal Verification Upon Request

Changes to C-II Rx - w/ Appropriate Verfication RPh May Add/Correct/Change Any Information on C-II EXCEPT:
* Patient’s Name
* Controlled Substance Prescribed
* Name/Signature of Prescriber
* Authorized Fill Date
