Report Each Rx to P(D)MP No Later Than: The Following Business Day
1) Prescription Information:
* Dispenser DEA #, Rx Date, Rx #/ID, Rx Origin (Form of Transmission)
2) Drug Information:
* Indicates New/Refill
* NDC #, Quanity, Length of Therapy
* Refill #, # of Refills Authorized
3) Patient Information:
* First & Last Name, DOB, Gender
4) Prescriber Information:
* Prescriber Name, DEA #, Date Issued
5) Pick Up Information:
* Who Picked It Up (Signature)
* Method of Payment
* If Not Dispensed - Submit Zero Report

Requirements for Reporting Suspected Fraud/Abuse (of ANY kind)?

Answer 2

Discovery of Controlled Substance Theft:
* Immediate Notice to the Board
* Fail to File = Board Determines Grounds for Discipline

Mandatory Reporter Training: COMPLETE WITHIN 6 MONTHS OF INTIAL EMPLOYMENT:
* Complete 2 Hours Adult & Child Abuse Identification & Reporting: 2 Hours Each (4 Hours Total)
* OR Complete/Maintain Adult & Child Abuse ID & Reporting: 1 CE Hour Each Every 3 Years (2 Hours Total)

Report to Vaccine Adverse Event Reporting System (VAERS): Within 24 hours of event, even if you are unsure it was caused by the vaccine.

If Licensee Fails to Notify Board of Violation:
