* If average number of prescriptions exceeds 150/day, must have on-site RPh 100% of the time and apply to the board for licensure as a general pharmacy within 10 days.
* Submit Limited Use/Telepharmacy Application & Nonrefundable Fee
* Submit CSA Registration Application & Nonrefundable Fee (If Dispensing Controlled Substances)
