Must RPh Be On-Site for Access to Pharmacy?
Yes, Unless Temporary Absence or if Pharmacist is on a Break:
* Temporary Absense (Under 2 Hours):
Keep log of who was designated to be in charge, all staff on-site, and activities performed while the pharmacy was closed.
* During Pharmacist Break (Refill Sales ONLY):
Pharmacist may delegate a technician to dispense refills that do not require counseling while pharmacist is on break.
Activities Prohibited in Pharmacist Absence:
* Dispensing / Counseling
* Final Verification
* Drug Use Review / Evaluating Patient’s Medical Record
* Decisions that Require Pharmacist Judgement
* Transferring Rx’s
After Hours Dispensing Allowed
Requirements (Dispensing Cabinet)?
Emergency Access to Pharmacy Procedures?
Hospital Deparment Closed:
* PIC shall identify, maintain, and document policies/procedures of the facility and which pharmacy staff has authorized access to the pharmacist department and which activities are authorized.
Staff Log Requirements?
Must Maintain Log of All Employees:
* Regardless if they are permanent, temporary, or intermittant staff.
* Must contain dates and shifts worked.
* Must be available for inspection and copying by board for a minimum of 2 years.
Identification Requirements to Access Pharmacy?
All Employees Must Wear While on Duty: Badge that includes title & at least their first name.
