What Information Must Be Provided for Phone Rx?

Oral Rx - all information requried for written Rx except prescriber address/signature:
* Date
* Patient Name/Address
* Drug/Device Name, Strength, Quanity
* Directions
* Prescriber Name (& DEA # - if applicable)
* Initials / Unique ID of RPh
* First Name, Last Name, & Title of Oral Rx Transmitter

Question 2

Who Can Phone in Prescriptions (Prescriber, Staff, Receptionist)?

Answer 2

Prescriber may authorize agent to send Rx order by phone, fax, or electronic transmission (must include first name, last name, & title)

Question 3

Can Technicians Receive New Prescriptions via Phone/Voicemail? Refills?

Answer 3

New Rx: No
Rx Refill: Yes, if technician trainee’s supervising pharmacist authorized that function.

Can Clerks/Cashiers/Others Receive New Prescriptions via Phone/Voicemail? Refills?
