Who must submit the form?

all biopsies and necrospies must be submitted through a veterinarian

Why must we submit a form?
A
- To record client details and constent
- identification of the sample
- provide information for the pathologist to aid interpretation

A
- signalment of the animal
- pertinent clinical history
- site of sampling and nature of the sample (using diagrams and photos)
- Macroscopic description of the lesion
- recent treatments
- suspected differential diagnosis

A
10% neuteral buffered formalin (used as soon as possible after excision)

What should you place on the label when shipping a sample?
A
- Patients name and owners name or patient identification number
- the site/source of the sample
- any information provided must correlate with the submission form

- in a sealed screw-top, leak proof container
- sealed plastic bag with sufficient absorbance material
- submission form should be outside the container
