Small for dates Flashcards
Define preterm delivery
24-36 weeks
Aetiology of pre-term delivery
Spontaneous Multiple pregnancy Infection Vascular- placental abruption Cervical incompetence Polyhydramnios (excess amniotic fluid)
Risk factors for pre-term delivery
previous pre-term delivery Conceived via IVF Age- teenagers are at increased risk Low BMI Uterine abnormalities Cocaine use, smoking, alcohol consumption
What weight is classed as small for gestational age
<10th centile
i.e. <2.5kg
what is Intra-uterine growth restriction (IUGR)
poor growth causing baby to be smaller than expected
Maternal causes of IUGR
smoker, alcohol consumption, drug use, age, hypertension, weight
fetal causes of IUGR
Infection (CMV, rubella) Congenital abnormality (absent kidneys, chromosomal abnormality)
placental causes of IUGR
Infarcts
Abruption
(often secondary to hypertension)
What is symmetrical IUGR
small head, small abdomen
What is asymmetrical IUGR
normal head, small abdomen
Consequences of IUGR in labour
risk of hypoxia
Consequences of IUGR post natally
Hypoglycaemia Hypothermia Polycythaemia Hyperbilirubinaemia Abnormal neurodevelopment Asphyxia
what is used to assess fetal heart beat
Cardiotocography
what indicates good reflex reactivity of fetal circulation on cardiotocography
accelerations
what is an acceleration
An increase in fetal HR at the start of the uterine contraction, returning to baseline before the start of the next
