RP - complete dentures opposed by natural teeth Flashcards
the presence of natural teeth means that excessive forces can be applied to the opposing edentulous ridge. what can this lead to?
trauma
increased resorption
lack of stability of dentures
what may trauma to the maxillary denture bearing area cause?
soft tissue damage; ulceration and discomfort
alveolar resorption and fibrous tissue replacement; ‘flabby ridge’ ‘fibrous ridge’ ‘combination syndrome’
what is ‘combination syndrome’ ?
lower anterior teeth occluding against edentulous upper ridge = resorption of anterior part of ridge and overgrowth of tuberosities
what is a clinical sign of a fibrous ridge?
tissues blanch when held
how would you take an impression of a fibrous ridge?
special tray with anterior perforations - mucostatic impression - shape of tissues at rest
how would you take an impression of a severe fibrous ridge?
2 stage silicone impression
special tray with anterior window
first imp with putty
cast away putty from fibrous ridge
inject light bodied material around fibrous ridge
what can be done to minimise alveolar resorption when there are still roots?
overdenture abutments
what is the effect of absence of lower posterior teeth and a full upper denture?
when occlude, stability is effected
posterior seal is broken
list ways to manage an irregular occlusal plane on the natural teeth?
no adjustment
minimal localised occlusal grinding
radical occlusal adjustment
extraction of teeth
overlay appliance
when would you consider using overlay appliances/ overdentures?
very worn teeth where FWS has been increased and OVD decreased
OH must be good as this is a plaque trap
what are issues associated with complete lower denture with upper natural teeth?
significant trauma to lower ridge due to:
- excessive occlusal forces
- occlusal imbalance
- minimal denture base foundation area
- accelerated resorption of lower ridge
why is trauma worse to lower denture with natural upper teeth?
lower denture has small base foundation so force is concentrated in a smaller area
what is the management options when there are issues with lower denture and upper natural teeth?
soft linings
retain roots and provide overdenture
implants
