what is sepsis?
an extreme body response to infection.
life-threatening medical emergency.
a pre-existing infection initiates a systemic sequence of events.
what gender is sepsis more common in and why?
males
females have a degree of hormonal response
what age is more at risk of sepsis?
over 75 years
what are some risk factors for sepsis?
surgery
broken skin
HIV/AIDS
cirrhosis
autoimmune disease
organ transplant
inflammatory disorders
what sites are at risk of sepsis?
lungs (64%)
abdomen (20%)
bloodstream (15%)
renal or genitourinary tract (14%)
what microbes are found in sepsis incidents?
47% gram positive 20% being s aureus
62% gram negative 20% being pseudomonas spp and 16% E coli
19% fungal
how do you recognise sepsis?
any patient presenting with a source of infection and 2 or more:
1. temp >38 or <36
2. HR >90 (high risk >130/min)
3. resp rate >20 (high risk >25)
4. WCC >12 or <4
5. BP systolic <100
what are sepsis red flags?
altered mental state/ confusion
unable to stand/ collapse
unable to catch breath/ speak
fast breathing
skin pale, mottles, ashen or blue
rash wont fade
recent chemotherapy
oliguria
what is the SEPSIS pnuemonic?
slurred speech
extreme shivering
passed no urine in a day
severe breathlessness
illness so bad feel theyre dying
skin mottles/ discoloured/ ashen
how do you manage sepsis?
- give 02
- take blood cultures
- give IV antibiotics
- give a fluid challenge
- measure lactate
- measure urine output
what is the BUFALO pnuemonic?
blood cultures
urine output
fluid resuscitation
antibiotics IV
lactate measurement
oxygen
