Oral med - pain of the cranial nerves Flashcards
what can pain attributed to lesion or disease of the cranial nerves be classified in to?
trigeminal neuralgia
painful trigeminal neuropathies
what can pain attributed to lesion or disease of the glossopharyngeal nerve be classified in to?
glossopharyngeal neuralgia
painful glossopharyngeal neuropathies
describe trigeminal neuralgia symptoms?
recurrent unilateral brief electric shock-like pains
abrupt in onset and termination
explain the aetiology of trigeminal neuralgia?
limited to the distribution of one or more divisions of the trigeminal nerve
triggered by innocuous stimuli
can develop without apparent cause or be a result of another diagnosed disorder
what is the diagnostic criteria of trigeminal neuralgia?
a. recurrent paroxysms of unilateral facial pain in the distribution of one or more divisions of the trigeminal nerve, with no radiation beyond, and fulfilling criterial B and C
b. pain has all of the following characteristics:
1. lasting from a fraction of a second to 2 mins
2. severe intensity
3. electric shock-like, shooting, stabbing or sharp in quality
c. precipitated by innocuous stimuli within the affected trigeminal distribution
d. not better accounted for by another ICHD-3 diagnosis
what is the incidence of trigeminal neuralgia?
4-13 per 100000
50-60 year olds
females > males
what can trigger trigeminal neuralgia?
dental tx or disease
chewing, talking, touching, cold
what can relieve trigeminal neuralgia in 65% of pts?
warmth and rest
what are the 3 types of trigeminal neuralgia and their causes?
classical TN - neurovascular compression
secondary TN - underlying disease
idiopathic TN - no apparent cause
what imaging can be used to investigate TN?
high resolution magnetic imaging
