Renal Cell Cancer Flashcards
Renal cell cancer is also known as…?
hypernephroma and accounts
RCC accounts for what percentage of Primary renal neoplasms?
85% of primary renal neoplasms
From what cells does RCC arise?
proximal renal tubular epithelium
Most common histological subtype of RCC?
clear cell (75 to 85%)
Risk factors associated with RCC
- middle-aged men
- smoking
- von Hippel-Lindau syndrome
- tuberous sclerosis
- autosomal dominant polycystic kidney disease
Classical triad of RCC
haematuria
loin pain
abdominal mass
RCC’s may have endocrine effects. What can they secrete?
- erythropoietin (causing polycythaemia)
- parathyroid hormone-related protein (causing hypercalcaemia)
- renin
- ACTH
What percentage of RCCs have metastasised at presentation?
25%
how do RCCs cause varicoceles?
- caused by the tumour compressing veins
- hence why majority are left-sided
Name the syndrome:
- Paraneoplastic disorder associated with RCC
- presents as cholestasis/ hepatosplenomegaly
- secondary to increased levels of IL-6
Stauffer syndrome
What stage is the following RCC?
Tumour ≤ 7 cm and confined to the kidney
T1
What stage is the following RCC?
Tumour > 7 cm and confined to the kidney
T2
What stage is the following RCC?
Tumour extends into major veins or perinephric tissues, but not into the ipsilateral adrenal gland and not beyond Gerota’s fascia
T3
What stage is the following RCC?
Tumor invades beyond Gerota’s fascia (including contiguous extension into the ipsilateral adrenal gland)
T4
Management of RCC
- Surgery - partial or total nephrectomy
- alpha-interferon and IL-2
- Receptor tyrosine kinase inhibitors (e.g. sorafenib, sunitinib)
