usually Streptococcus pyogenes.
What causes post-strep glomerulonephritis?
immune complex (IgG, IgM and C3) deposition in the glomeruli
What age group is most commonly affected?
Young children
Clinical features
headache
malaise
visible haematuria
proteinuria
maybe oedema
hypertension
oliguria
Findings on blood tests in post-strep glomerulonephritis
- raised anti-streptolysin O titre
- low C3
differentiate IgA nephropathy from Post-strep glomerulonephritis
IgA
- young male
- 1-2 days post URTI
- macroscopic haematuria
Post-strep
1-2 weeks after
children
low complement
haematuria but may also have protein/oedema
Features of post strep GN on renal biopsy
- endothelial proliferation with neutrophils
electron microscopy:
- subepithelial ‘humps’ caused by immune complex deposits
immunofluorescence:
- granular or ‘starry sky’ appearance
