What is epididymo-orchitis?
A
infection of the epididymis +/- testes

Most common symptoms
A
unilateral pain and swelling
may have urethral discharge

Most common infective causes
A
local spread from genital tract
- Chlamydia
- Gonorrhoeae
Local spread from bladder
- E-Coli

most important differential if a patient presents with testicular pain/swelling
A
TORSION
- needs to be excluded urgently to prevent ischaemia of the testicle.

Factors which would suggest torsion > infection
A
- Age < 20 years
- Severe pain
- Acute onset

Investigations to determine the infective cause
A
- STI screen
- Urine for culture

Management
A
If STI most likely:
- refer to sexual health clinic
- unknown organism
=> ceftriaxone IM + oral doxycyline for 10-14 days
If enteric organism most likely:
- 2 weeks of quinolone
