IgA Nephropathy Flashcards
What is IgA Nephropathy also known as?
Bergers disease
How does IgA nephropathy typically present?
- Frank (Visible) haematuria
- in young people
- following an upper respiratory tract infection (URTI)
Give examples of conditions associated with IgA Nephropathy
alcoholic cirrhosis
coeliac disease/dermatitis herpetiformis
Henoch-Schonlein purpura
Pathophysiology
- mesangial deposition of IgA immune complexes
Findings on histology
mesangial hypercellularity
positive immunofluorescence for IgA & C3
Nephrotic range proteinuria is rare in IgA nephropathy TRUE/FALSE
TRUE
Renal failure is common in IgA Nephropathy
renal failure is unusual
only seen in a minority of patients
Differentiating between IgA Nephropathy and Post-strep glomerulonephritis
Both occur following URTI and present with haematuria
Post-strep GN
- occurs 1-2 weeks post URTI
- low complement levels
- proteinuria
IgA Nephropathy
- occurs 1-2 days post URTI
- young males
- frank haematuria
Management of IgA Nephropathy
isolated hematuria + minimal proteinuria + normal GFR
= no Tx needed
persistent proteinuria/ slight fall in GFR
- ACE inhibitors
Active disease (e.g. falling GFR) or failure to respond to ACEi
- steroids
What percentage of patients with IgA Nephropathy develop end stage renal failure?
25%
Is frank haematuria a sign of good or poor prognosis in IgA Nephropathy?
good prognosis
Markers of poor prognosis in IgA Nephropathy
- male
- large proteinuria
- hypertension
- smoking
- hyperlipidaemia
- ACE genotype DD
